The original music was in MIDI format. By the Lord Blackthorn's Revenge expansion, it was removed and replaced entirely by MP3.

There were 60 tracks in the game at public launch, many of them reused from prior Ultima games. There are currently 91 music tracks in the game.
Music in Ultima Online was originally generated using MIDI files. They were provided in three versions. The first was a General MIDI file, and produced the lowest quality sound. The other two used SoundFonts, which were basically instrument samples, to play the MIDI, and generated a more pleasing sound. The SoundFont MIDIs came in 512 and 4MB versions, depending on the amount of memory your soundcard had.
With the release of Ultima Online: Lord Blackthorn’s Revenge, the music was upgraded to MP3s. In the process, many of the original MIDI songs were replaced with completely different songs. These original MIDI songs are no longer available in the game.
Most current music can be played in game using Dawn's Music Box. 15 additional tracks were added to be played with Harpsichords.
The MIDI recordings included here are courtesy of the Ultima Digital Music Archive, or are direct gstreamer conversions from the midi files. Other good resources are Rabbit's Lair, where Christian K. provides recordings of the non-soundfont MIDIs played with professional MIDI equipment (a Roland Sound Canvas 55), and the archived Stratics page of Techarin's MP3 Music Hall, which includes custom-orchestrated versions of the SoundFont midis made by Dinaris Techarin.

The music from Ultima Online has, for the most part, never been released outside of the game itself. However, versions of some songs have been released on soundtracks for other Ultima games. The only official soundtrack release was the "Ultima Online 10th Anniversary Original Sound Track", released only in Japan with purchases of Game Time 100+ in 2010.

This CD actually did not contain the Ultima Online soundtrack, but instead consisted of tracks from Ultima IX: Ascension and Ultima X: Odyssey, with the addition of Elf City from Ultima Online. Most of the Ultima IX and X tracks also appear in Ultima Online, but the following tracks on the CD do not:
